

1) You may not go to sites deemed dangerous by the US State Deparment (http://travel.state.gov/travel/cis_pa_tw/tw/tw_934.html)
2) You must obtain LLU Insurance prior to submitting the GO Peds Application form to your GO Peds director.
3) Your GO Peds director will have a discussion with your on-site faculty prior to approving your application.
4) You will not receive your monthly salary unless the trip is either a LLUMC faculty-led trip or an Adventist site.
5) You are responsible for routing the GO Peds application to the Pediatric Residency Office, Pediatric Department Chairman, Housestaff office and returing the form to Ann Elliott in A1120A for filing. You must also indicate that you have submitted the forms for LLU Insurance/malpractice
6) Sign the rules for GO Peds sheet.

1) You must send an e-mail once weekly to the GO Peds director regarding your condition and what you are learning
2) Your supervisor must send an e-mail to me each week.
3) Follow the guidelines for BEST PRACTICES FOR SHORT-TERM HEALTHCARE MISSIONS (http://healthcaremissions.org/BESTPRACTICES.html

1) You must submit a brief bibliography of the four required papers that you read. Submit 20 lines for each reference.
2) You must submit a 3-5 page report of your experience with 2-3 photos.
3) You must submit your supervisor's rotation assessment
4) If you want to be re-imbursed for your airfare you will submit items 1, 2, 3 from above and proof or your airfare expenses.
